Healthkeeperz welcomes Nahaven Jacobs, BSN, RN, CHPN, as clinical manager of hospice care

Healthkeeperz is proud to welcome Nahaven Jacobs, BSN, RN, CHPN as the new clinical manager of the Hospice Care Program. Jacobs assumed the role Jan. 5, a return that represents both professional growth and a meaningful full-circle moment.

Jacobs’ journey with Healthkeeperz began years ago in the pharmacy, where she worked from November 1996 through May 1997. Those early experiences guided by the kindness of Mr. Howard Brooks, the strong yet nurturing leadership of Mrs. Brenda Brooks and the steady encouragement of Dr. John Brooks left a lasting impression. Jacobs often recalls being reminded, “You can always do better,” a message that helped shape her career and calling.

Now, Jacobs returns with a wealth of clinical experience, including work in critical care, cardiac specialty nursing, post-anesthesia recovery and more than nine years in hospice care. She earned her Certification in Hospice and Palliative Care in September 2024.

“Nahaven brings an incredible depth of clinical experience, compassion, and leadership to our hospice program. Her heart for patients and families, combined with her knowledge of hospice operations, will strengthen our team and the care we provide across our communities,” shared Director of Hospice Care Kellie Britt in a press release.
